A Framework for Change

Numerous challenges are thrown at us throughout our journey in this life. Challenges lead to crossroads, choices, and change. Navigating life with diabetes thrown into the mix can be more complex, but recognising opportunity, making choices that feel right for you, will positively influence your outcomes.

A diagnosis.
A moment of worry.
A conversation that lingers.
A New Year’s resolution that quietly fades away (or simply doesn’t happen!)
Or simply the slow realisation that something isn’t working anymore.

These are all potential crossroads – times when you pause, look at your life, and start to wonder about change.

Crossroads & Choices is a reflective framework to support you through those moments. It isn’t a programme. It isn’t a checklist. And it certainly isn’t about perfection or “fixing” yourself.

It’s intended to be a tool that you can use to help recognise and navigate opportunities for change, whilst focusing on what matters most to you, and what small, realistic choices feel possible right now.



How Crossroads & Choices works

1 Noticing the Crossroad (Pre-contemplation)

  • Recognising the opportunity.
  • What has brought me here?
  • What’s shifted, unsettled, or asking for my attention?
  • Decision to commit to change.

This might be practical, emotional, cultural, medical, spiritual, or relational. All of it is valid.

2 What Matters Most to YOU? (Contemplation)

  • What do I value?
  • What do I want more of in my life?
  • What do I need to learn more about?
  • What helps me feel grounded, connected, or well?

Your answers don’t have to be perfect or polished. They just need to be yours.

3 Understanding the Reality (Determination)

  • Planning in your real life context.
  • What’s really happening in my life right now?
  • What pressures, responsibilities, health issues, or barriers am I carrying?
  • What strengths and supports do I already have?

This stage is about honesty – not judgement.

4 Exploring the Choices and Testing the Water

Given my values and my reality…

  • What choices might be possible for me right now?

These might be tiny shifts – or simply a change in perspective.

5 Choosing the Next Small Step and Taking Action

  • What feels doable?
  • What feels kind?
  • What supports the direction I want to grow in – without overwhelming me?
  • Taking intentional action.

Small steps still move you forward. Often, they are the most sustainable ones.

6 Maintenance – Practising Self-Compassion Along the Way

  • Check-in and evaluate or re-evaluate
  • Is this working?
  • Is this sustainable?

If things don’t go to plan (and sometimes they won’t)…

  • Can I respond with kindness instead of criticism?
  • What have I learned from this moment?

Compassion is the foundation – not the reward.


This resource is for anyone who…

  • feels stuck or overwhelmed
  • is navigating health, wellbeing, or life changes
  • lives with diabetes or supports someone who does
  • has “tried everything” and is tired of being told to try harder
  • wants a more human, values-based approach to change

You are the expert in your own life.
This framework exists to walk alongside you, not direct you.


A note about culture, identity, and context

Our values, whānau, culture, spirituality, health experiences, trauma, responsibilities, and histories all shape our crossroads and our choices.

Crossroads & Choices honours that.

This isn’t about forcing yourself into someone else’s idea of success.
It’s about finding the path that fits your life — your body — your world.
